I dont know what you mean by entity and its legal definition.

The medical billing company is the business associate of the provider

Also, I dont know the business or contractual arrangement you have with this server service. if it is just to store your data for you to use, they may not need a BA agreement just for storing your data.  Everything is based on your contract with them and what they do for you.  A simple non-disclosure agreement may all that is needed. However, I am not a HIPAA Lawyer, which could be to whom you may need to speak with.
