Hi guys.  Sorry I was not clear in my earlier post.  I have drown up some forms that I would need to have a new client fill out when contracting to ourtsource their billing.  Some of the forms include: Business Associate Agreement and a Service Contract.  Also a form to have the group practice Tax ID #, telephone number, address, fax #, etc. and to have each providers name and NPI #.

Danni R. was exactly on target when "guessing" what I was asking.

My question was are there any other forms you may suggest to have or any other information to include in these forms?
