Just FYI, I ask these questions because they provide a foundation to an appeal.  I can show when the service was provided, when the claim was sent, if there is a contract, that it was sent within the contracted timeframes. States are important to know Statutory claim submission timeframes in case the provider is not contracted.

I watched 200 episodes of Law and Order to learn this.

if the DOS is March 31, how can you send a claim on March 15???

Questions:

1) What was the date of service.

2)  What date was the claim sent.

a)  Was the claim sent by paper or electronic.

3) Is the provider contracted?

If yes,

What is the contract timeframe? 

4)  Where was the service provided (which state)

You cant do anything without the answers to these questions.
