U people took soap in the wrong direction Wink. Soap notes are also the notes which are mentioned on the packing of any soap, which descrieb who, when and where u can apply that soap......think u all are pretty much familiar how to use a soap ... Grin

yesterday, I spent all day in the bathtub and ran out of soap. The package said to use thoroughly, rinse and repeat.  I did that about 30 times when I used it all up.  Now I have to get more and the same thing will happen again.

Oh, I love our guys here. You are the BEST! So funny.

The bath was great. Soaked and red a clive cussler novel

finished Dan Browns Angels & Demons. Cant Wait for the movie

For S*O*A*P  I use Irish Spring bath wash with aloe

Helps keep the leg and chest scars soft

Soap is how providers documented the care visit

S: Subjective = What is wrong with the patient.  What brings the patient to you today and history of the problem

O:  Objective:  The ordering and review of tests, examination of the patient and examination findings.

A: Assessment:  The diagnosis or disagnoses

P: Plan:  The treatment regimen. 


It has now been replaced by History, Examination and Medical Decision Making.

New Note:  To clear the air.  HEMDM is for E/M. 

S = History
O = Examination
A = MDM & Dx

I said to myself, I used soap for injuries I treated.  I must have done more than 146,000 soap notes in my 20 years of patient treatment in the Army and many were for injuries as well. 

Some doctors still use SOAP. It works when the documentation components of an E/M are present in the documentation.
