You check "CHAMPUS" and you make a photo copy of the TRICARE / TRICARE PRIME insurance card, as well as the Military ID card. These photo copies must be added to the patient's medical record file.
TRICARE is valid ONLY with the Military ID card, whether active or retired, and the Military ID card must not be expired.
For the records, you enter the "sponsor's
SSN number", not the Military ID card number.
